Financial Policy
Co-pays and cosmetic procedure fees are required at the time of service. Forms of payment accepted include cash, checks, MasterCard, Visa, and American Express.
Whenever a growth is removed from the skin, the tissue will be automatically sent for pathology, unless otherwise indicated by the doctor. This will result in two separate charges when a biopsy is performed. One will be from your dermatologist for removing the sample. The other will be from the pathologist for examining the sample under the microscope. In most cases, both charges will be billed at your dermatology office visit.
Cancellation Policy
If you are unable to keep your appointment, we ask that you kindly provide us with more than 24 hours of notice. This courtesy will make it possible to give your appointment to another patient.
Cosmetic and Surgical Cancellation Policy
It is necessary to reserve longer appointment times for surgical and cosmetic procedures. If you miss your appointment or do not provide us with at least 24 hours notice of your cancellation, a $300.00 fee will be charged to your account.
Appointment for Minor Children
A parent or legal guardian MUST accompany minor children 18 years of age or younger for their initial appointment. Any follow-up appointments made for a minor child must either be accompanied by parent or legal guardian, or minor child must bring an "Authorization to Treat Letter" signed by the parent or legal guardian. The letter should provide a contact phone number in the event the physician or office staff need to contact the parent or legal guardian. You may use the form we have provided in our "New Patients" area of our website.
Identification/Insurance Cards
All new patients are asked to present a valid form of identification such as drivers license, identification card, passport, or military identification card. Patients will also need to present their current insurance card. It is the patient's responsibility to bring current insurance information in order for us to be able to process a claim. If insurance information is not available at the time of visit, the patient may be responsible for payment in full at the time of service.
